Prepay for Scheduled Initial In-Home Assessment
Thank you for scheduling an in home Initial Assessment. Due to the exclusivity of this appointment (we cannot put anyone else in this time slot) and the cost of travel, we must ask that you prepay the $300 Initial Assessment fee prior to confirming your appointment. This is done by clicking on the button to the right and paying by PayPal or Major Credit Card.
This fee is 100% refundable provided you cancel with at least 24 hours notice. Also, there is no fee for rescheduling if done with at least 24 hours notice. If you reschedule with less than 24 hours notice, we require a $150 rescheduling fee. This fee is again prepaid.
Rescheduled Initial In-Home Assessment ($150)
Telephone Consulting - 30 minutes intervals ($75)
You will have been instructed to order or pay for your telephone consulting by clicking on the button to the right and paying by PayPal or Major Credit Card. This fee is 100% refundable provided you cancel with at least 24 hours notice. Also, there is no fee for rescheduling if done with at least 24 hours notice. If you reschedule with less than 24 hours notice, this fee is nonrefundable.
PC Moderator Purchase ($150 +shipping)
You will have been instructed to order or pay for your PC Moderator and its initial device setup. Click on the button to the right and pay by PayPal or major credit card. Before ordering, make sure you have chosen the correct model. The model is dependent on the system configuration. Click on the link to the right to help you decide if you are not aware of the model required. This purchase is refundable (less shipping costs) within 30 days of purchase.
